Vendor Guidelines

The Vendor Reference Guide (VRG) is designed to provide a quick reference into the general vendor database for reporting and other possible opportunities to work with the HACP. The (VRG) is not to be considered a certification of ability / offer of engagement / nor acceptance of exception to any other general HACP requirements for vendor participation. 

** All items denoted are required by The Housing Authority of the City of Prichard

• Copy of professional resume of projects 
• Prior work references
• Liability & Workmen's Compensation Insurance Coverage and amount 
• City Of Prichard Business License (copy) 
• Automobile Insurance Coverage (copy) 
• Tax I.D. number or Social Security # (copy)
• Web/ site or e-mail address (if available)
• Driver's license # and (copy) 
• Complete the w-9 form (completed before engagement) 
• Pease include any additional information that will support your certification or validation of your skills or services as stated herein
