The Housing Authority of the City of Prichard (HACP) is currently accepting applications for the following position:

ELDERLY SERVICE COORDINATOR

REQUIRED SKILLS, KNOWLEDGE AND ABILITIES: Provide general case management including intake and referral services to all residents requesting and/or needing such assistance. Establishes linkages with appropriate agencies and service providers in the general community. Sets up a directory of providers for use by both Community staff and residents. Refers and links the residents to service providers in the community; for example, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, healthcare services, preventative health screening/wellness, substance abuse and legal advocacy. Educates residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate. Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual. Manages the provision of supportive services where appropriate. Set up volunteer support programs with service organizations in the community. Helps the residents build informal support networks with other residents, family and friends. May provide training to residents in the obligations of tenancy or coordinate such training. May assist other staff on issues related to “Aging in Place” and service coordination. Sponsors educational events which may include subjects relating to health care, job search seminars, life-skills training, safety issues, etc. Work as a team member with the Property Manager for the property to run smoothly there must be communication between the Property Manager and the Elderly Service Coordinator. Meet on a regular basis with the Property Manager to provide updates on planned educational sessions, agency meetings and/or schedule changes. Inform the Property Manager of any resident issues that could adversely affect that resident’.

MINIMUM QUALIFICATIONS:

Must possess a bachelor’s degree from an accredited college or university with major coursework in social work, sociology, psychology or related field and a minimum of five (5) years of experience in social work or case management.

ANNUAL SALARY: $ 40,000.00
